Job summary

AccentCare Inc. is seeking a Registered Nurse Case Manager in Pennsylvania to oversee patient care plans from admission through discharge in Chester and Delaware Counties. The role blends direct and indirect patient care while guiding RNs, LPNs, and CHHAs to ensure the plan of care is delivered.

Responsibilities include initial assessments, accurate documentation, and timely communication with physicians. On-call and full-time schedule with competitive pay per point and annual compensation

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assessment of nursing needs and management of patient care plans.
• Complete initial patient assessment upon admission with history.
• Select nursing diagnoses and plan interventions.
• Review medications for safety and interactions.
• Set realistic, measurable goals aligned with diagnoses.
• Inform physicians of assessments and recommended interventions.
• Implement nursing interventions within Nurse Practice Act.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes timely.
• Communicate changes in patient condition to physicians.
