Job summary

Peoples Health Services is seeking an experienced Registered Nurse for home health in the Florida Panhandle. The RN will assess patients, develop and implement a Plan of Care, and coordinate with the clinical team to ensure quality, cost-effective outcomes.

The role requires strong communication, knowledge of PPS, OASIS, and case management, with supervision of LPNs and aides as needed. A Florida RN license is required.

Qualifications

• Current Florida RN license.
• Experience in home health is preferred.
• Familiar with PPS and care coordination.

Responsibilities

• Performs admissions, resumption of care, and follow-up using OASIS data collection.
• Develops Plan of Care based on assessment findings and guidelines.
• Coordinates care with the interdisciplinary team to promote positive outcomes.
• Documents clinical notes and plan of care in a timely, accurate manner.
• Supervises LPNs and Home Health Aides to ensure plan fidelity.
