Amedisys Home Health in Lynchburg, VA is seeking an experienced registered nurse case manager to join our home-based care team. This role supports patients in their homes, coordinating care with the clinical team and providing teaching to promote independence and well-being.

The RN will conduct assessments, plan care, supervise LPNs and HHAs, and participate in on-call rotations. The Baylor schedule (Fri/Sat/Sun) is available, with competitive benefits and opportunities for growth.

Qualifications

• 1+ year of RN clinical experience.
• Current RN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Documents care timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• On-call responsibilities and services to patients/families as assigned.
• Participates in clinical development and continuing education programs.

Qualifications

• One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
