AccentCare in San Diego is seeking an RN Case Manager, Home Health. This full-time, on-site role covers the East County region.

You will coordinate patient care, develop care plans, and collaborate with a multidisciplinary team to deliver high-quality home health services.Qualifications include RN licensure in the state, at least 1 year RN experience, CPR certification, and a valid driver's license with reliable vehicle to travel between locations.

• Graduated from an approved school of professional nursing and licensed as an RN in the state of operation.
• At least 1 year of RN experience.
• CPR certification required; maintain current certification.
• Must be a licensed driver with a reliable vehicle and able to travel to all business locations.

If you meet these qualifications, we want to meet you!

• Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
• One (1) year experience as a RN.

Required Certifications and Licensures:

• Licensed to practice as a registered nurse in the state of agency operation.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
