Full Time- Home Health Roanoke Region.As part of the health care team, assumes primary accountability for managing nursing care and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care.

  • Interacts with patients, families, visitors, and all members of the health care team to develop, coordinate, and manage the plan of care. Manages and oversees delivery of patient care such as and not limited too:
  • Maintains patient, staff and customer confidentiality. Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient's status. Interprets the appropriate information needed to identify each patient's requirements relative to his or her age specific needs and provides care according to departmental policies and procedures. Work involves application of advanced knowledge and skills of patient care. Duties are varied and complex. Accountable to resolve issues as determined by license. Handles routine issues, follows up to conclusion on matters, while referring only the most complex or higher level issues and problems to the Manager, Team Lead, or Supervisor. . Must be able to work in a fast paced environment with the ability to effectively prioritize and demonstrate multi-tasking skills. Impact of work affects CHCS and Carilion Clinic scorecard initiatives. Strong critical thinking skills are necessary and needed to interpret laws and regulations related to CHCS and Carilion Clinic. May be exposed to infectious diseases, bio-hazardous and hazardous materials, including regulated medical wastes, and combative patients. Work is performed in a patient care environment and is often fast-paced, requiring use of prioritization skills.
  • Physical requirements include: lifting, prolonged periods of standing, bending, pushing, pulling, reaching, walking, etc. May be asked to provide care at other Carilion facilities to meet patient needs. Some positions may be required to take call. May be required to travel in adverse weather. Care provided in a variety of home settings. What We Require:

Education: Graduate of accredited Registered Nurse Program.

Experience: One year of experience preferred,

Licensure, certification, and/or registration: Currently licensed as Registered Nurse in the Commonwealth of Virginia or new graduate of approved program eligible to sit for the Virginia licensure exam. For Home Health, must have valid Virginia Drivers License and pass a Criminal Record Check as per Virginia state law. Must obtain /maintain AHA BLS-HCP.

Other Minimum Qualifications: Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions.
