Job summary

Health Here seeks an RN Case Manager to provide comprehensive primary care to 25-35 homebound patients, coordinating multidisciplinary services to address physical, emotional, psychosocial, and educational needs.

The role includes assessing patient needs, revising care plans, overseeing care from admission to discharge, and supervising staff while ensuring regulatory compliance and accurate documentation. Weekend/on-call rotation flexibility is required.

Qualifications

• RN licensure (single or multi-state) and BLS certification.
• A valid driver’s license with private vehicle insurance.
• Minimum 1 year of Medical/Surgical nursing experience; home health and ICU experience preferred.
• Knowledge of home health operations, reimbursement, laws, and OASIS is preferred.

Responsibilities

Role:

Responsibilities:

They assess patient needs, develop and revise individualized care plans, manage patient care from admission to discharge, supervise LPNs and Home Health Aides, and ensure goals are met. They communicate with physicians, team members, patients, and families, providing education to promote self-care and independence. Additionally, they ensure compliance with regulatory guidelines, complete documentation accurately and promptly, participate in quality improvement activities, and support staff development.

Qualifications & Skills:

Candidates must possess an Associate’s or Bachelor’s Nursing degree, RN licensure (single or multi-state), BLS certification, and a valid driver’s license with private vehicle insurance. A minimum of 1 year of Medical/Surgical nursing experience is required, with preferred experience in Home Health and ICU. Strong critical care, assessment, communication, leadership, organizational, and computer skills are essential. Knowledge of home health operations, reimbursement, laws, and OASIS is preferred.

Additional Expectations:

The role involves participating in performance improvement, attending ongoing education, and adhering to all policies and standards. Flexibility to perform weekend and call rotations is also required.
