Job summary

Providence at Home with Compassus is seeking a registered nurse to plan, implement, and direct home health services in the Portland West area. You will work with physician, patient/family, and referring agencies to coordinate care in the patient’s home.

The role emphasizes case management, supervision of the Interdisciplinary team, adherence to policies, infection control, and on-call coverage, with a compensation range of $53.27–$74.30 per hour and comprehensive benefits.

Qualifications

• Graduate of an accredited nursing program with RN licensure.
• Current, unencumbered State RN license.
• Current driver's license.
• CPR certification with TB screen and Hepatitis consent/declination.

Responsibilities

• Assess home health patients to identify care needs.
• Case management and clinical oversight to the interdisciplinary team.
• Implement and revise the plan of care.
• Apply infection control measures to protect staff and patients.
• Supervise and direct HHA/LVN to ensure quality care.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payor requirements.
• Provide on-call service to patients and families.
• Communicate effectively with patient/family and care team.
• Adhere to company policies and information security policies.

Position Summary

Primary functions are to plan, develop, implement, evaluate, organize and direct home care services with the interdisciplinary team including physician, patient/family, and referring agency; and administer skilled nursing care for clients of all ages in their place of residence and assume the responsibility for the total provision of care.

Essential Duties And Responsibilities

• Assesses home health patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations
• Case manages and provides clinical oversight and direction to the interdisciplinary team, physician, and family
• Implements/develops/documents the plan of care and revises as necessary
• Provides care utilizing infection control measures that protect both the staff and the patient (OSHA)
• Supervises and provides directions to the HHA/LVN to ensure quality and continuity of services provided
• Assures continuity of quality patient care delivered with appropriate documentation
• Monitors assigned cases to ensure compliance with requirements of third party payors
• Performs on-call responsibilities and provides on-call service to patients/families, as assigned
• Provides effective communication to patient/family, team members, physicians, and other health care professionals
• Adheres to all company policies and procedures.
• Adherence to and compliance with information systems security is everyone’s responsibility. It is the responsibility of every computer user to: Know and follow Information Systems security policies and procedures. Attend Information Systems security training, when offered. Report information systems security problems.

Education/Licensing/Certification

Minimum Qualifications

• Graduate of an accredited college/university, Associate, Diploma, or Baccalaureate School of nursing
• Current, unencumbered State License as a registered Nurse
• Current Driver’s License
• Current CPR, negative TB screen and Hepatitis consent/declination

Experience

• Two years of experience as a registered nurse in a clinical care setting, home health preferred.

Knowledge And Skills

• Nursing skills as defined as generally accepted standards of practice.
• Good interpersonal skills.

Compensation

The current compensation range for this position is $53.27-$74.30 per hour. The specific rate of pay offered to a selected candidate will be determined based on relevant education, training, experience, geographic location, and other position-related factors. Compassus does not inquire about, consider, or rely upon an applicant's prior salary history in determining compensation. In addition to base compensation for the position, an employee working in this position may be eligible for pay differentials based on hours, shifts worked, or other criteria. They may also qualify for bonuses under applicable bonus programs or at Compassus’s discretion, as well as other forms of variable pay under relevant company policies. A summary of applicable pay programs will be provided during the offer process.
