comforcarecareers is seeking a compassionate Registered Nurse Case Manager to support non-medical home care clients. This role focuses on client assessments, care plan oversight, and caregiver supervision, not skilled nursing or hands-on clinical care.

The RN Case Manager will help ensure clients receive safe, appropriate, and personalized care from our team of professional caregivers, with emphasis on in-home assessments and ongoing care planning.

Qualifications

• Active RN license in good standing.
• Prior home care, senior care, case management, or assessment experience preferred.
• Strong communication and documentation skills.
• Comfortable completing in-home assessments.
• Ability to build positive relationships with clients, families, caregivers, and office staff.
• Reliable transportation and valid driver’s license.
• Compassionate, dependable, and detail-oriented.

Responsibilities

• Complete initial and ongoing client assessments in the home.
• Develop and update non-medical care plans based on client needs.
• Oversee and support caregivers assigned to client cases.
• Monitor client condition and communicate changes to office leadership and families as needed.
• Ensure caregiver services align with the approved care plan.
• Provide guidance and coaching to caregivers on client-specific needs and expectations.
