Regional One Health is seeking a Case Manager/Clinical Leader to assess and address the physical and psychosocial needs of patients and families, coordinating with clinical and social services across hospitals, homes, and community settings.

The role emphasizes developing community-based care plans, managing a complex caseload, and driving improvements in outcomes while collaborating with a multidisciplinary team to remove barriers to access and ensure continuity of care.

Qualifications

• Bachelor’s Degree in Nursing is required.
• Master’s Degree in Nursing or Public Health is preferred.
• Education, training, or experience to work with the patient population is required.

Responsibilities

• Designer/coordinator/integrator/evaluator of care for individuals, families, groups, communities, and vulnerable populations.
• Demonstrates ability to understand rationale for care and deliver interventions in diverse environments.
• Oversees holistic assessment and develops a community-based plan of care.
• Carries a caseload of complex patients.
• Synthesizes data to evaluate health outcomes and reports on interventions.
• Advocates for patients and collaborates with professional teams.
• Educates clients, families and staff using current information and teaching strategies.
• Maintains individualized care plans addressing medical and social barriers to accessing care.
• Coordinates and integrates activities within complex care and departments to maximize department effectiveness.
