Job summary

Constellation Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to join our team. This field-based role covers an assigned territory, providing in-home care to adults and geriatric patients in homes and facilities.

You will perform head-to-toe assessments, oversee medications and wound care, and develop plans of care with other clinicians. OASIS knowledge is highly valued, with daily reporting to the Clinical Manager and iPad-based documentation.

Qualifications

• Registered Nurse licensed in the state of employment.
• 1–2 years of RN experience, home care preferred.
• OASIS experience strongly desired.
• Must have reliable transportation and be willing to commute to provide in-home patient care.
• Some weekend days are required; summer and winter holiday commitments as needed.

Responsibilities

• Provide skilled nursing and head-to-toe assessments for adults and geriatrics.
• Oversee medication administration and wound care per physician plan.
• Deliver home health visits to an assigned caseload including admissions, re-certifications and discharges.
• Develop Plan of Care with health care professionals.
• Educate patients and caregivers on diseases, meds, plan of care, and treatment.
• Report to Clinical Manager daily.
• Document using an iPad within required time frame.

Constellation Home Care is looking for a compassionate and skilled Registered Nurse (RN) Case Manager to join our team!
