Pay range:
Hourly $51.79 - $77.69
The Case Manager applies the process of assessment, planning, implementation, monitoring, evaluation, and coordination of care with the goal of ensuring a safe and sustainable discharge from the inpatient setting to the most appropriate level of post-acute care. By integrating evidence-based clinical standards, preventive guidelines, protocols, and other metrics in development of patient-centric treatment plans, the Case Manager promotes quality and efficiency in the delivery of care. The Case Manager also uses the nursing process to provide education and resources to facilitate informed decision-making and timely access to post-acute care. Works in collaboration with the health care team to ensure safe, quality and cost-effective care.
