About the Role:
The Rn Case Manager coordinates and facilitates safe, timely, and appropriate patient discharges by assessing needs, developing individualized discharge plans, and collaborating with patients, families, healthcare providers, community agencies, and post-acute care facilities. Arranges follow-up services, equipment, medications, transportation, and community resources while addressing barriers to discharge. Maintains accurate documentation, communicates effectively with interdisciplinary teams, and ensures continuity of care and compliance with organizational regulatory requirements.

Strong communication skills are essential for coordinating with multidisciplinary teams and educating patients and families about treatment options. Organizational skills enable the management of multiple cases simultaneously while ensuring compliance with documentation and regulatory standards. Proficiency with electronic health records and case management software supports efficient tracking of patient progress and facilitates data-driven decision-making. Additionally, problem-solving abilities help the RN Case Manager address barriers to care and optimize resource utilization to improve patient outcomes.

Hours: M-F 8:30 AM-5:00 PM.
This is a FT position budgeted at 40 hours per week. This position, if applicable is eligible for overtime oay.
