Jobot is seeking a Full-time Hospice RN Case Manager in Queens, NY. The RN handles a small territory (3 zones/ZIPs) and will see four hospice patients daily, delivering skilled nursing care in home and facility settings, coordinating hospice services per the plan of care.
Responsibilities include care planning, patient/family education, collaboration with physicians and the IDT, and comprehensive documentation. NY RN license required; BSN preferred; 15K sign-on and robust benefits included.
Full time Hospice RN role with 15K Sign on Bonus, PENSION and 403B Retirement plan and 4 weeks vacation
