Job summary

Mass General Hospital, part of Mass General Brigham, is seeking a Registered Nurse Case Manager to support the Case Management department. You will coordinate care across the continuum, assess patient needs, and advocate for efficient and high-quality outcomes.

The role requires an MA RN license and at least 2 years of Med/Surg experience, with a case management background preferred. Onsite work at 55 Fruit Street with a standard 40-hour schedule may apply.

Qualifications

• Must be a licensed RN in the state of Massachusetts.
• Minimum of 2 years Med/Surg experience required.

• ADN required, but BSN strongly preferred.

The Case Manager, as a member of the health care team, collaborates to enhance the delivery of patient care services along the continuum of care. The Case Manager meets patient's needs efficiently and expeditiously by continuously improving the patient's experience, helping to ensure the institutional standards of high-quality patient care, reducing cost, and ensuring reimbursement. Through broad knowledge of clinical care and systems management, the case manager evaluates, predicts, and facilitates the trajectory of patient care. The case manager employs a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet the individual's health care needs.
