Interim HealthCare Inc. is seeking a Registered Nurse Case Manager (RNCM) for the South Hills area. This full-time role involves performing in-person assessments and coordinating home health services to ensure quality care.

You will manage care plans, educate patients and families, and work with a team of caregivers to reduce readmissions through preventative care. Competitive pay and benefits are offered.

Qualifications

• RN licensure in the state where the position is based.
• Current CPR/AED/BLS/First Aid certification required.
• One year of RN experience in home health or similar setting preferred; OASIS experience preferred.

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments.
• Oversee and update the patient care plan with caregivers, RNs, and LPNs.
• Communicate patient conditions and coordinate with providers to deliver care.
