Job summary

Amedisys is seeking an experienced Registered Nurse case manager to coordinate home health and hospice patient care in the Murfreesboro area. You will assess patients, develop care plans, and collaborate with physicians and the care team to ensure high-quality outcomes.

You’ll guide families and caregivers, document progress, and participate in on-call coverage as needed. A valid RN license, CPR certification, and reliable transportation are required.

Qualifications

• One year of nursing experience.
• Current RN licensure in applicable state(s).
• CPR certification is required and maintained.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Makes referrals to hospice care disciplines as needed or documents rationale for not doing so.
• Develops and maintains patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes and outcomes as appropriate.
• Teaches patients, families and caregivers patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services as assigned.
• Other duties as assigned.
