Job summary

Girling Health Care of New York, part of the Valence Care family, is hiring CHHA RN Case Managers for our Brooklyn Office. You will manage home health nursing services, coordinate with physicians, and ensure quality patient care in accordance with CMS/DOH guidelines.

The role requires NYS RN license, BSN preferred, CHHA experience, and strong communication skills. Starting salary $110K–$115K with opportunity for growth in a supportive team environment.

Qualifications

• BSN preferred or equivalent nursing degree
• Current RN license in NYS required
• CHHA experience required
• Spanish/Chinese language proficiency preferred
• Minimum 1 year of medical/surgical nursing experience
• Experience with EMR and basic computer applications (Word, Excel) preferred
• Strong organizational and communication skills
• Ability to manage multiple cases and deadlines
• Confidentiality and professional conduct maintained

Responsibilities

• Assess adults and geriatric patients in home health settings
• Plan and manage provision of professional and ancillary home health services
• Update care plans with patient/caregiver input and physician coordination
• Complete documentation per CMS/DOH policies
• Coordinate with multidisciplinary team for care and authorization
• Monitor quality of care and participate in QI activities
• Discharge planning in collaboration with the care team
• Maintain compliance with corporate and regulatory guidelines
