The Case Manager, as a member of the healthcare team, collaborates to enhance patient care services along the continuum of care. They meet patient needs efficiently by improving the patient experience, ensuring high-quality care, reducing costs, and facilitating reimbursement.
Using broad clinical and systems management knowledge, the case manager evaluates, predicts, and facilitates patient care trajectories. They employ assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet individual healthcare needs.
We offer exciting opportunities for RNs in the Case Management department, which provides comprehensive orientation and onboarding, as well as an innovative "Novice to Expert" program.
