DaVita Integrated Kidney Care (IKC) is seeking a Full-Time RN Case Manager (RN) to join our team in Elk Grove, CA.
As a Registered Nurse (RN) Case Manager, you will support complex patients, helping them navigate healthcare challenges. Your responsibilities include medical record review, identifying patient needs across medical, social, emotional, and financial domains, and implementing interventions. You will work independently and collaboratively to ensure quality, cost-effective care.
Location: Hybrid-Remote (work from home and travel within the assigned area for in-person visits at dialysis clinics and nephrology practices).
Schedule: Monday-Friday, with flexibility to accommodate patient and partner availability.
Position: Full-time.
Key responsibilities include building empathetic relationships with patients and families, coordinating care, managing complex cases, addressing social determinants of health, and assessing medical conditions.
