Job summary

OhioHealth in Ohio is seeking a Care Manager to partner with patients, families, and providers to deliver organized, cost-effective, patient-centered care. The role collaborates with interdisciplinary teams to promote health through prevention and management strategies.

Responsibilities include applying evidence-based interventions across populations to improve quality measures, patient satisfaction, access to services, and reduce hospital/ED utilization.

Qualifications

• RN from an accredited school of Nursing with a current unencumbered RN or APP-RN licensure in the State of Ohio.
• Bachelor's degree and 2-5 years recent nursing practice experience with knowledge of acute hospital, community or public health nursing concepts and resources.
• Computer skill and data entry.
• Knowledge and skill in clinical assessment and disease management.
• BLS issued by American Heart Association.
• Associates hired after 1/1/2020 must have their Bachelor’s degree in nursing.

Responsibilities

• The Care Manager works in partnership with patients, families, caregivers and providers to deliver targeted care that is organized, coordinated, cost effective and patient-centered.
• Collaborates with interdisciplinary staff and stakeholders within the care continuum to promote patients’ optimal health through disease prevention and management strategies.
• Apply evidence-based interventions to populations for cost efficient and outcome-based care.
