Job summary

CommonSpirit Health's CHI St. Vincent in Arkansas is seeking an experienced RN Case Manager to drive coordinated patient care across the hospital stay and beyond.

You will assess, plan, and facilitate comprehensive discharge planning, advocating for patients while collaborating with physicians, nursing staff, insurers, and post-acute providers to ensure timely transitions. Requirements include RN licensure in Arkansas, a master's degree in Case Management or Nursing, CPR certification, and 2+

Qualifications

• Graduate of an accredited school of nursing and a minimum of two years of acute hospital clinical experience.
• Masters in Case Management or Nursing and at least 1 year of experience.
• Registered Nurse license for Arkansas.
• Basic Life Support (CPR) certification within 90 days of hire.

Responsibilities

• Completes discharge planning assessments and documents findings.
• Facilitates multidisciplinary discharge planning with patients, families, and post-acute providers.
• Oversees and evaluates the implementation of the discharge plan.
• Collaborates with the care team to ensure appropriate inpatient resource use.
• Ensures timely and smooth transitions to post-acute services based on clinical needs.
• Identifies readmission risk and implements mitigation strategies for at least 30 days.
