Job summary

Baptist Health in Kentucky seeks a Full-Time Case Manager to coordinate and complete discharge planning and care transitions across the continuum of care. You will manage a caseload, perform timely assessments, plan and evaluate care, and ensure efficient use of resources in line with CMS requirements.

This role requires an RN license in Kentucky, BSN or equivalent experience, and at least three years of relevant experience.

Qualifications

• BSN required or in lieu of BSN degree, a combination of education and clinical experience would be acceptable
• Three years of relevant experience required
• Active KY RN License required

Responsibilities

• Coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans.
• Manage a designated caseload across the continuum of care.
• Facilitate communication among care providers and other stakeholders.
• Ensure CMS and external review compliance in care coordination.
