Job summary

Houston Methodist Willowbrook Hospital is seeking a PRN Case Manager RN to collaboratively plan and coordinate care for a designated patient population on assigned units. You will work with physicians and the health care team to ensure clinically appropriate admission, discharge planning, and meaningful care transitions, while maintaining quality and cost-effective outcomes.

Responsibilities include utilization management, chart review, and communication with payors; you will document in EPIC

Qualifications

• RN with hospital nursing experience and case management exposure.
• Three years hospital nursing experience including two years in case management.
• Ability to communicate effectively with patients, families, and care teams.

Responsibilities

• Collaborates with physicians and interprofessional team to plan and manage designated caseload; ensures plan of care is patient-focused and discharge-ready.
• Performs utilization management and reviews for medical necessity and appropriate level of care.
• Documents in EPIC and MIDAS; participates in performance improvement and education of staff.
