Job summary

Heart to Heart Hospice in Texas is seeking an RN Case Manager who will assess and coordinate care for patients facing life-limiting illnesses, supporting families with compassionate guidance.

The role emphasizes comprehensive initial and ongoing assessments, development of individualized plans of care, and collaboration with an interdisciplinary team to ensure compliant, patient-centered care. On-call responsibilities and timely documentation are required.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate Nursing program.
• Current State RN license in good standing.
• Two years of RN experience (healthcare/clinical) preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs across physical, psychosocial, and environmental factors.
• Implement the individualized plan of care and revise as needed.
• Collaborates with patient, caregivers, physicians and healthcare professionals.
• Attend IDG meetings and coordinate patient care planning and priorities.
• Perform on-call responsibilities and provide on-call support to patients and families.
• Ensure timely submission of documentation and compliance with regulations.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN preferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
