Job summary

UAB Medicine in Birmingham, AL seeks an RN for Case Management (Days). The nurse coordinates the interdisciplinary plan of care from admission to discharge and creates a comprehensive care plan that honors patient and family preferences while monitoring medical necessity and level of care.

The role collaborates with the care transition team to ensure effective implementation and reevaluation of the health plan, and ensures proper documentation and post-discharge services.

Qualifications

• BLS Provider within 30 days of hire date.
• RN license eligible or licensed by the Alabama Board of Nursing.

Responsibilities

• Coordinates the interdisciplinary plan of care from admission to discharge.
• Creates care plan across continuum aligning with patient/family preferences.
• Monitors medical necessity and level of care via assessments and records.
• Advocates for resources and removes barriers to care.
• Maintains dialogue with supervisor and care transition team for implementation and reevaluation of health plan.
• Serves as resource for medical record documentation and post-discharge services.
