Overview

We are seeking a Case Manager to join our team.

Hospital Details

Vibra Hospital of Central Dakotas, located in Mandan, ND, is a 41‑bed critical care hospital providing long‑term acute care services to patients recovering from serious illnesses or injuries. Our patients often require additional critical care for complex conditions such as trauma, infectious diseases, wound healing, cardiovascular disease, stroke, amputation, and ventilator weaning. As a smaller, specialized hospital, we offer an opportunity to work in a fast‑paced and friendly environment where you can truly get to know your patients and their families.

Participants in the Clinical Career Ladder are eligible for pay increases following successful completion of the Novice, Intermediate, and Advanced levels.

Responsibilities

The Case Manager is responsible for coordinating clinically complex patients’ care across a continuum; ensuring and facilitating the achievement of quality clinical and cost outcomes, negotiating, procuring, and coordinating appropriate services and resources needed by the patients, and at key points, intervening to address and resolve issues or concerns. The role ensures appropriate sequencing of treatment goals and implements a comprehensive discharge plan. The Case Manager demonstrates knowledge and skills necessary to provide case management services to the patient population experiencing rehabilitation needs.

Required Skills

• Current, valid, and active license to practice as a Registered Nurse, MSW, or Licensed Social Worker in the state of employment.
• Current BLS certification from a Vibra‑approved vendor.

Additional Qualifications

• One year of case management experience preferred.
• Previous LTAC experience preferred.
• CCM or ACM certification preferred.
• Working knowledge and ability to apply professional standards of practice in job situations.

• Comprehensive knowledge of the principles and concepts of case management and applicable standards of voluntary and regulatory agencies for review activities.
