$52.57/hr - $52.57/hr

REGISTERED NURSE - CASE MANAGER

Location:JOINT BASE MCGUIRE-DIX-LAKEHURST (JBNJ) Lakehurst, NJ

Schedule: Monday-Friday, 0730-1630 (1-hour unpaid lunch)

Vaccine Requirement: COVID vaccine not required

Pay Rate: $48/hour + $4.57/hour for Health & Welfare- =$52.57 hr.

Key Responsibilities

• Provide case management for diverse populations (adults, children, families, seniors)
• Perform assessment, planning, coordination, implementation, and evaluation of care
• Collaborate across disciplines and advocate for patient care
• Travel may be required to inspect military housing for patient accommodations
• Attend meetings, contribute to program evaluation, and maintain documentation in accordance with DoD and AF policy
• Train and orient staff, contribute to committees
• Integrate utilization management and maintain community liaison relationships
  
  
  

Required Qualifications

• Education:
• ADN or BSN from a program accredited by a US Department of Education-recognized agency
• Experience:
• ADN: Minimum 3 years full-time clinical case management
• BSN: Minimum 2 years full-time clinical case management
• Must have experience with core CM functions: assessment, planning, facilitation, advocacy
• License:
• Active, unrestricted RN license in any U.S. state or jurisdiction
• Certifications:
• Certified Case Manager (CCM or ANCC), or certification-eligible based on experience
• BLS Certification
• Skills:
• Proficient in Microsoft Office and internet use
• Familiar with HIPAA, AAAHC, and Joint Commission standards
• Strong communication, organizational, and problem-solving skills
• Able to interact with difficult, distressed, or fearful individuals tactfully
