Formación

• BSN degree from an accredited program
• Active and unencumbered RN license
• 1+ years in case management healthcare setting
• Strong interpersonal and written/verbal communication
• Proficiency in EHR software

Responsabilidades

• Conduct comprehensive patient assessments including medical history and psychosocial needs
• Collaborate with providers, patients and families to develop care plans
• Monitor progress and adjust care plans as needed
• Serve as main contact between patient and providers for care coordination
• Maintain up-to-date records and confidentiality/compliance
