Incredible Health in Fresno, CA is hiring a Nurse Case Manager (RN) to join our hospital partners. You will conduct comprehensive patient assessments, collaborate with clinicians and families, and coordinate individualized care plans to ensure high-quality outcomes.

We value strong communication, experience in case management, and proficiency with EHR systems. Shifts include day and night, with full-time and part-time opportunities; benefits include health coverage, 401(k), and PTO.

Qualifications

• BSN or higher from an accredited program
• Active and unencumbered RN license
• 1+ years experience in case management, preferably in healthcare or hospital setting
• Exceptional interpersonal and communication skills to collaborate with medical professionals, patients, and families
• Proficiency in electronic health records (EHR) software

Responsibilities

• Conduct comprehensive assessments of patients, including medical history, medications, treatment plans, and psychosocial needs
• Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans
• Monitor and evaluate patient progress, adjusting care plans as needed and communicating changes to the healthcare team
• Serve as main point of contact between patient and healthcare providers to ensure seamless communication and care coordination
• Maintain up-to-date records and case notes, adhering to confidentiality and compliance standards
