• BLS Certification is required.
• Must have 3 years of acute care experience as a Nurse.
• Previous case management experience preferred.

Responsibilities

• Utilizes Interqual or the 2MN rule to determine status for all patients.
• Collaborates with interdisciplinary teams for care progression.
• Assesses and implements strategies to reduce barriers in care transitions.
• Follows CMS & regulatory requirements for Case Management.
