Job summary

SolutionHealth seeks an RN Case Manager to join the Care Coordination team at Elliot Hospital in Manchester, NH. The role focuses on clinically-based case management to ensure safe transitions and optimal resource use. On-the-job training is available for experienced nurses meeting the requirements.

You will collaborate with the care team to create transition plans that address clinical and social determinants of health, coordinating with community providers for a smooth discharge.

Qualifications

• Bachelor’s degree in a healthcare-related field or equivalent experience.
• Current BSN or commitment to achieving the degree in a specified amount of time.
• CCM or ACM certification preferred.
• Experience with case management and documentation in healthcare settings.

Responsibilities

• Assess and identify patients’ needs during discharge planning.
• Provide resources and community referrals for continuity of care.
• Coordinate care with families, providers, and clinical team.
• Follow Medicare conditions when needed.
• Conduct utilization reviews.
