Job summary

Tufts Medicine is seeking a per diem Case Manager to oversee patient care across the continuum, coordinating services and engaging patients and families in care decisions to minimize fragmentation and improve outcomes. Requirements include an Associate Degree in Nursing, BLS, RN license, and three years of case management experience; a Bachelor's Degree in Nursing is preferred.

Strong communication and collaboration with an interdisciplinary team are essential.

Qualifications

• Completes assessments on all new admissions to determine the need for Case Management services.
• Conducts daily multidisciplinary rounds on assigned units and coordinates the care plan.
• Discusses estimated length of stay and discharge plan with the attending physician and healthcare team.
