Job summary

Manatee Memorial Hospital in Bradenton, Florida, part of Universal Health Services, is seeking a Case Manager to coordinate patient care across the continuum with a focus on discharge planning and utilization management. You will collaborate with physicians, social workers, and payers to optimize outcomes and minimize denials.

RN licensure in Florida is required; case management certification is preferred. You will contribute to data collection and reporting while promoting efficient health

Qualifications

• High School Diploma or GED required.
• Associate’s degree preferred; Bachelor’s degree preferred; Master’s degree not required.
• Florida RN license required.
• Case Management Certification preferred.

Responsibilities

• Lead multidisciplinary planning and coordination of patient care across the healthcare continuum.
• Coordinate discharge planning and intervention with the clinical team and providers to promote wellness and efficient utilization of health services.
• Communicate outcomes and data to leadership; participate in reporting and quality initiatives.
• Support care across the continuum with collaboration among physicians, social work, and payers.
• Perform other duties as assigned.
