Job summary

UNC Health Nash Hospitals is seeking an experienced RN Case Manager to support the NGH Case Management unit. You will assist the Director, organize departmental activities, evaluate case management processes, and provide clinical leadership at the bedside.

Responsibilities include coordinating case management for a unit, serving as a clinical resource for patients and staff, and assisting with priority setting and discharge planning to optimize length of stay and care transitions.

Qualifications

• RN license in North Carolina is required.
• 3 years of Healthcare/Medical nursing experience.
• BCLS and ACLS certifications currently held.

Responsibilities

• Assist the Director with organizing departmental activities and evaluating the case management process.
• Coordinate and provide case management to an assigned unit based on staffing needs.
• Serve as a clinical resource for patients, staff and other departments.
• Assist nursing staff with priority setting and discharge planning.
