Job summary

• RN license in Oregon within 6 months of hire and credentials from multiple states as required.

Responsibilities

• Proactively build effective relationships by providing resources and guidance to coworkers and members.
• Complete tasks autonomously using up-to-date subject-area expertise and ensure policies are followed.
• Assess medical necessity for hospital admission and appropriate level of care with minimal supervision.
• Coordinate resources to assure continuity and quality of care, review authorizations, and participate in case management rounds.
• Develop client-focused case management plans with treatment goals in collaboration with patients, families, and healthcare team.
• Attend seminars and stay updated on policies and community resources; monitor data and identify improvement opportunities.
