Job summary

The position requires collaboration with physicians and social workers, flexibility for rotating shifts, and adherence to state laws. Experience in case management and knowledge of funding resources are preferred.

Qualifications

• Two years of professional experience in a clinically related area.
• Dependent upon the department, transportation for site visits may be required.

Responsibilities

• Coordinate case management from entry to post-discharge across outpatient settings.
• Identify patients suitable for intervention based on cost, complexity, or requests.
• Monitor and document quality of care; assess outcomes and care plan effectiveness.
• Determine appropriate level of care using diagnosis, severity and standards.
• Discuss funding and insurance issues with patient, family, and providers.
• Negotiate with third party payers on benefits, eligibility and utilization.
• Coordinate discharge planning with care team and resources.
• Set measurable discharge goals and promote self-management for patients.
• Oversee CMS Important Message issuance to patients.
• Supervise technical support staff on case management teams.
• Arrange follow-up appointments and perform post-discharge calls as needed.
• Possibly conduct home or site visits; work rotating shifts including nights/weekends.
