Job summary

Beth Israel Lahey Health in Massachusetts is seeking an RN Case Manager to coordinate care across medical, surgical, ICU and outpatient areas. Qualifications

• Associate's degree in Nursing required; Bachelor's preferred.
• Licensed Registered Nurse (RN) required.
• 3–5 years of related acute care or managed care experience; two years in case management.

Responsibilities

• Performs utilization management, care coordination, and discharge planning for assigned caseload.
• Reviews medical records for severity of illness and intensity of service using InterQual criteria to meet regulations.
• Develops case management care plans using assessment, planning, implementation and evaluation.
• Facilitates multidisciplinary collaboration daily.
