Responsibilities

• Perform nursing process (assessment, care planning, intervention, evaluation) within a patient-centered practice model.
• Oversee implementation of an interdisciplinary plan of care with an individualized discharge plan.
• Reevaluate plan of care based on patient needs and conditions.
• Engage in committees, task forces and staff meetings to influence clinical practice and decisions.
