Job summary

Loma Linda University Health is seeking a skilled Case Manager RN to join a Magnet-recognized academic medical center in California. You will coordinate interdisciplinary care plans, ensure appropriate levels of care, and work from admission through discharge using evidence-based nursing processes within a collaborative team.

Qualified candidates hold a BSN, CA RN license, and at least two years in hospital nursing with case management or discharge planning experience; BLS is required.

Qualifications

• BSN required; minimum two years of hospital nursing experience.
• Minimum two years of case management, utilization review, or discharge planning in acute care preferred.
• Active and unrestricted California RN license; BLS certification required.

Responsibilities

• Oversee implementation of an interdisciplinary plan of care from admission to discharge.
• Assess, plan, implement and evaluate patient care within the nursing process.
• Ensure medical necessity criteria are met for the level of care provided.
• Collaborate with the care team and participate in governance and staff meetings.
