Job summary

WellSpan Health seeks a part-time Utilization and Case Management Nurse to apply utilization and case management techniques to optimize patient care and resource use. You will work in a team model with social work to address discharge needs and lead integration of utilization principles.

RN licensure required with 2 years of acute care experience. Collaboration across care teams is essential to coordinate discharge planning, DME, home health, IV antibiotics, and cost-effective quality care.

Qualifications

• Associates Degree required; bachelor's preferred.
• 2 years acute care experience required.
• Experience in utilization management or case management preferred.
• Licensed RN upon hire; multi-state license preferred.
• Basic Life Support required.

Responsibilities

• Reviews patients for medical necessity and setting appropriateness.
• Coordinates progression of care to optimize throughput.
• Liaises with utilization management, payers and treatment team.
• Assists with discharge planning and alternative care settings.
• Identifies cross-continuum case management needs and collaborates.
• Supports value-based care initiatives across the system.
