Under general supervision, you will assess patient needs, monitor care from admission to discharge, and support cost-effective, high-quality outcomes while adhering to Milliman criteria and relevant billing regulations.

Qualifications

• Graduate of an accredited School of Nursing required.
• RN license and BLS certification required at time of hire.
• Professional Case Management certification preferred.
• Bi-lingual English/Spanish preferred.

Responsabilités

• Monitor utilization from admission to discharge to ensure cost-effective care.
• Assess patient needs and coordinate post-hospital resources with the care team.
• Collaborate with interdisciplinary team to ensure appropriate discharge planning.
• Apply Milliman Criteria and maintain documentation per policy.
• Coordinate discharge arrangements with social workers and family.
