Qualifications
- Licensed RN in the state you work in.
- Proficiency with EMR software.
- Excellent planning, organizing and communication skills.
- Ability to solve problems autonomously while maintaining high standards.
- Valid driver’s license and reliable vehicle with current insurance.
Responsibilities
- Conduct initial client assessments and refine care plans.
- Provide hands-on nursing care and patient education.
- Maintain open lines of communication with patients, families, and the healthcare team.
- Manage patient care coordination for your caseload.
- Participate in interdisciplinary team meetings to ensure comprehensive care planning.
