DaVita Integrated Kidney Care (IKC) is seeking a Full-Time RN Case Manager in the South Austin, TX market.
As a DaVita IKC Registered Nurse (RN) Case Manager, you will support complex patients, helping them navigate the healthcare system. Responsibilities include medical record review, consultation, and addressing medical, social, emotional, and financial needs of CKD/ESKD patients, implementing appropriate interventions. You will work independently and collaboratively to ensure quality, cost-effective care.
Location: Hybrid-Remote (work from home with travel for in-person visits within assigned geography).
Schedule: Monday - Friday, flexible to accommodate patient and nephrology team availability.
Position: Full-time.
Key duties include building empathetic relationships, coordinating care transitions, managing complex/at-risk patients, and addressing social determinants of health.
Requirements include a current RN license (BSN preferred), at least 2 years RN experience with healthcare coaching, CPR/BLS certification, valid driver’s license, willingness to travel, and computer proficiency.
Preferred qualifications include 2+ years in Case Management or Chronic Care, utilization management experience, and Spanish language skills.
