The RN Case Manager is responsible for managing care coordination for members of a Medicare Advantage plan, including visiting patients, coordinating with healthcare teams, and ensuring proper nursing care.

This position requires a self-starter, team player, with excellent communication skills, ability to manage multiple priorities, and adaptability to change.

Qualifications:

• Minimum 2 years of clinical nursing or geriatric rehabilitation experience
• 2 years managed care experience
• 3-5 years case management in a managed care environment (preferred)

Essential Functions:

• Conduct health risk assessments
• Develop and update care plans
• Maintain caseloads, coordinate patient care, and provide education
• Coordinate discharge planning and participate in audits and quality improvement projects
• Research clinical questions and educate members on health access
• Respond to care coordination issues effectively

Education & Licensure:

• Graduate of an accredited RN program
• Bachelor’s Degree (preferred)
• Current RN license in the state
• CPR certification
• Valid driver’s license and insurance
• CCM certification (preferred)

Experience:

• Required: 2 years in geriatric care
• Preferred: 2 years in skilled nursing
