Overview

The Case Manager Registered Nurse (RN) provides comprehensive and coordinated care to patients. Working under moderate or general supervision, this role analyzes patient needs, participates in care coordination, conducts research, reconciles information, and reviews care plans.

Responsibilities
  1. Analyzes patient cases to assess their needs and develop individualized care plans.
  2. Prepares and processes documentation related to patient care to ensure accuracy and regulatory compliance.
  3. Researches and stays current with evidence-based practices and healthcare guidelines relevant to the patient population.
  4. Reviews and evaluates the effectiveness of care plans, monitors patient outcomes, and adjusts care as needed.
  5. Performs other duties as assigned.
Qualifications
  • Education: Required: Associate's Degree; Preferred: Bachelor's Degree.
  • Experience: Required: 3 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or other nursing organizations.
