Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to lead each patient’s hospice care team and develop/adjust plans of care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

The RNCM will provide direct patient care, administer medications, document care, and coordinate with physicians and the multidisciplinary team to ensure comfort and quality of life for patients receiving hospice services. On-call and rounding duties may apply.

Qualifications

• Current RN license in state(s) of practice.
• Nursing degree from an approved program and licensure in state(s) of practice.
• Ability to pass DHS background, consumer report, and drug test.
• Valid state driver’s license and access to reliable transportation.

Responsibilities

• Lead patient care as part of hospice care team and develop Plan of Care with Hospice Medical Director, patient, family, and facility staff.
• Provide day-to-day patient care, administer medications, and maintain accurate records.
• Coordinate care with physicians, interdisciplinary teams, and community partners.
• Educate patients and families on self-care, medications, and diet.
• Ensure HIPAA compliance and maintain patient confidentiality.
• Participate in on-call visits, meetings, and supervise ancillary personnel.

The RN Case Manager provides day-to-day patient care, manages treatment, administers medications, maintains patient records and directs other members of the patient care team to meet the changing needs of the hospice patient, with a focus on providing comfort and enhanced quality of life.Essential Functions and SkillsPatient CareProvides care in accordance with the Nurse Practice Act and professional nursing standards.Conducts comprehensive patient and family assessments to determine hospice needs, including physical assessments and health history.Develops, implements, and revises the Plan of Care based on ongoing patient evaluations.Administers medications and treatments as prescribed in the physician’s plan of care.Educates patients and families on self-care, medications, and diet as appropriate.Maintains accurate clinical documentation, recording pain management, symptoms, and changes in condition.Coordinates care with physicians, interdisciplinary teams, and community health partners.Ensures a safe environment for patients and supports their activities of daily living.Provides emotional support and counseling to patients and families.Arrange for necessary medical equipment and supplies.Ensures HIPAA compliance and patient confidentiality.Additional DutiesParticipates in on-call visits as required including evenings and weekends.Attend interdisciplinary group meetings.Supervises ancillary personnel and delegates responsibilities as needed.Engage in professional development through training and continuing education.Contributes to quality assessment and performance improvement activities.Performs other duties as assigned.Requirements/QualificationsMust have a current Registered Nurse (RN) license in state(s) of practice.Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact.Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice.Ability to pass DHS background, consumer report, and drug test.Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.Preferred1+ years of recent experience in hospice/home care preferred.2+ years of Nursing experience, preferredPhysical RequirementsThe physical requirements described are representative of those that must be met to successfully perform the essential responsibilities of this position. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
