The RN Case Manager monitors and manages patient care to promote continuity, outcomes, satisfaction, cost efficiency, and compliance.
Responsibilities include consulting with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of care, participating in discharge planning, providing education and resource referrals, performing chart reviews, and other duties as assigned.
May be required to perform duties of CNA, MA, LPN, or other nursing staff occasionally.
Attendance at staff meetings, adherence to policies, and working scheduled shifts including overtime and call schedules are expected.
Minimum Education: Associate's degree.
Required Skills: Critical thinking, decisive judgment, ability to work independently, stress management.
Certifications: AHA Basic Life Support (BLS).
Licenses: RN licensed by the State of Tennessee or a Compact state with proper Tennessee licensing requests submitted.
