Highpoint Health with Ascension Saint Thomas, a Joint Commission-accredited hospital serving south-central Tennessee with locations in Winchester and Sewanee, has an opening for a Registered Nurse to join our Case Management Team.

About Winchester, TN

Job Responsibilities

• Monitor and manage patient care to ensure continuity, optimal outcomes, satisfaction, cost efficiency, and compliance.
• Collaborate with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of medical care, including admissions, transfers, and discharges.
• Respond to patient, family, and payer inquiries regarding care and satisfaction.
• Participate in discharge planning and coordinate community resources and patient transfers.
• Provide discharge education and resource referrals.
• Perform chart reviews to identify issues related to service delivery, outcomes, satisfaction, compliance, and reimbursement.
• Perform other duties as assigned, including occasionally covering CNA, MA, or LPN roles.
• Attend staff meetings, in-service trainings, and participate in committees.
• Adhere to hospital policies, procedures, and standards of behavior.
• Work scheduled shifts, including overtime and call schedules as required.

Minimum Qualifications

• Associate's degree required.
• Critical thinking, decisive judgment, and ability to work independently.
• Ability to work effectively in stressful environments.

Certifications and Licenses

• AHA Basic Life Support (BLS) certification required.
• Valid Registered Nurse license in Tennessee or a compact state with proper request submission for Tennessee licensing.
