About the Role

As a Registered Nurse Care Manager (RNCM) at Cityblock, you’ll lead care coordination for a panel of rising- and high-intensity members. You’ll partner closely with a Community Health Partner (CHP) to co-design care plans, track progress, and deliver integrated, person-centered care for members living with complex medical and behavioral health needs.

Key Responsibilities

• Serve as the clinical lead for your panel of members, initiating care plans and care coordination upon engagement.
• Conduct assessments including self-efficacy tools, condition-specific screeners, and behavioral health screeners (PHQ-9, GAD-7, AUDIT, DAST-10).
• Perform in-person clinical examinations as needed and collaborate with internal and external teams to determine care intensity.
• Lead and participate in multidisciplinary case conferences to ensure continuity and alignment of care plans.
• Co-create care plans with members, incorporating medical, behavioral, and social needs in partnership with the CHP.
• Conduct regular clinical check-ins and follow-ups, ensuring that therapeutic goals are met and adjusted as needed.
• Manage chronic conditions and support behavioral health interventions within the RN scope.
• Provide medication reconciliation, administration, education, and adherence support.
• Close prioritized quality gaps by documenting and coding accurately (ICD/CPT) and leveraging available tools and reporting dashboards.
• Navigate EHR and scheduling platforms to log care interactions, organize data, manage tasks, and coordinate with other Cityblock and community-based resources.
• Act as an advocate for members, removing barriers to care and supporting them in achieving their goals.

Requirements

• Education: Graduate of an accredited school of nursing (R.N.)
• Licensure: Registered Nurse with active Virginia RN license
• Experience: Minimum of 3 years of nursing experience, preferably in community-based, case management, or value-based care settings
• Strong interpersonal and communication skills to build trust with diverse populations
• Ability to work both independently and collaboratively with interdisciplinary teams
• Comfort using technology platforms including EHRs and communication tools

Preferred Qualifications

• Experience working with medically and socially complex populations
• Behavioral health familiarity and comfort using validated screening tools
• Previous experience in home-based care or field-based roles
