Qualifications

  • Bachelor's Degree (Required).
  • RN licensure in the State of Ohio (unencumbered).
  • APRN license in the State of Ohio.
  • BLS Certification from American Heart Association.
  • 2–5 years of recent nursing practice with knowledge of hospital or community health concepts.
  • Computer skills and data entry.

The Care Manager collaborates with interdisciplinary staff and stakeholders within the care continuum to promote patients’ optimal health through disease prevention and management strategies. Evidence-based interventions are applied to populations for cost efficient and outcome-based care. Outcomes include disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates.

Minimum Qualifications:
  • Bachelor's Degree (Required)
  • APRN - Advanced Practice Registered Nurse License - State of Ohio Board of Nursing
  • BLS - Basic Life Support - American Heart Association
  • RN - Registered Nurse - Ohio Board of Nursing
  • RN from an accredited school of Nursing with a current unencumbered RN or APP-RN licensure in the State of Ohio.
  • Bachelor's degree and 2-5 years recent nursing practice experience with knowledge of acute hospital, community or public health nursing concepts and resources.
  • Computer skill and data entry.
  • Knowledge and skill in clinical assessment and disease management.
  • BLS issued by American Heart Association.
